I was reviewing some logs today and noticed that the word ‘combined’ was showing up as every line of the log file, rather than the actual combined log data. It turns out I had accidentally commented out the line that defines what combined means:
#LogFormat "%h %l %u %t \"%r\" %>s %b \"%{Referer}i\" \"%{User-Agent}i\"" combined
